Health and Dental Plan
In September 2006, the Graduate Student Association assumed control of its own Health and Dental Plan. Our insurance broker is Gallivan and Associates, and the company which provides our insurance benefits is Great West Life.
All registered Full time Graduate students are automatically enrolled on the Graduate Student Association Health and Dental Plan. The Dental plan is optional, however, proof of equivalent coverage must be provided to opt out of the Health plan. Both plans have a policy year of September 1 through to August 31. Both plans offer spousal/family coverage for an additional fee.
HEALTH PLAN:
The annual fee is $189.
Your plan covers 80% of most medications legally requiring a prescription. The Plan is based on the Provincial Drug Benefit Formulary, and covers generic equivalent drugs where available. The following provides information on a few specific items.
Coverage is provided for:
Most insulin, needles, syringes and chemical diagnostic aids for the treatment of diabetes, Most oral contraceptives and a variety of paramedical services including; chiropractor, naturopath, clinical psychologist, speech therapist, and physiotherapist.
DENTAL PLAN:
The annual fee is $176.
To briefly summarize, coverage is provided for:
100% of one examination per year, including most x-rays, based on the current fee schedule 100% of one cleaning and polishing, at one unit only and includes up to 4 units of scaling (above the gum
line).
100% of one fluoride treatment per policy period 75% coverage of most fillings 75% coverage of extractions, limited to 2 wisdom teeth in any calendar year $500 annual maximum per policy period, per person. Before having major dental work completed, please contact Gallivan and Associates to ensure coverage
How Do I Opt Out?
You may opt out of either the health and/or dental plan.
DENTAL PLAN
The dental plan is optional, so you may opt out of this without providing proof of alternative coverage.
To opt out of the dental plan you must complete a waiver form. These are available in the GSA lounge located in Room 203, Ganong Hall. Completed forms can be returned to the GSA lounge and will be mailed on your behalf.
YOU WILL BE REQUIRED TO OPT OUT OF THE DENTAL PLAN ON AN ANNUAL BASIS.
HEALTH PLAN
To opt out of the health plan you must complete a waiver form and You must provide confirmation of your existing health coverage in order to opt out of the health plan (not including New Brunswick Medicare or Medicare equivalent). These are available in the GSA lounge located in Room 203, Ganong Hall. Completed forms can be returned to the GSA lounge and will be mailed on your behalf.
Your health opt out will carry forward as long as you remain an eligible Graduate Student. You may reverse your health opt out from the previous year before the opt out deadline.
DEADLINES
4 PM Friday September 25, 2009 or 4 PM Friday October 30, 2009 for MBA students.
In order to opt out of the health plan, documentation must be supplied as confirmation of having coverage (i.e. pay direct card or statement from your insurer). You may also choose to coordinate your benefits with your existing plan to get up to 100% coverage.
How Do I Add On Family Members?
You may also add a spouse or dependant onto the plan by completing a family add-on form. This must be done before 4 PM Friday October 16, 2009.
